SUMMARY OF KEY FUNCTIONS

A Community Health Supervisor (CHS) will be posted at CHCs both without delivery and with delivery services in urban areas. The CHS will supervise all community health activities, not only Urban CHW activities. He/ She will assist   to Urban Community Health Workers (CHWs) and support and supervise them. CHSs also supervise public health programs and promote collaboration between the facility and community. He/ She also assists in the formation and linkage of Community Health Shura (Shura-e-Sehie) and the Urban CHW program/ health facilities. He/ She is responsible for supporting the community in identifying and addressing their health problems.

KEY RELATIONS

Reports to Community Engagement Specialist
Collaborates with UHI project  
Actively contributes to Urban health initiative (UHI) project to ensure that the project interventions are implemented in effective and efficient manner and the project goal and objectives are met.

Location: Kabul(District 11,12,13 &15), Mazar-e-Sharif(District 4,5,6&9) Herat(District 5,6,10&12) and Kandahar(District 3,9,10&11)

                                                          KEY RESULT EXPECTED

Conduct training for CHWS and health promoters

Assist CHW trainers to conduct the theoretical training courses for Urban CHWs, and supervise the activities of the Urban CHWs during their practices in the community
Provides on-the-job and monthly in-service training to Urban CHWs
Assists in identifying Urban CHW training needs.
Designs and implements plans to upgrade the skills of both Urban CHWs and members of the community Health Shura, as needed.

Other relevant tasks:

Assists the staff of the health facility in making plans for the community health programs in the facility and its catchment area.
Implements and supervises the community health program activities in the catchment area of the health facility.
Identifies and reports immediately to the head of the health facility any problems that may interfere in achieving program objectives
Guides the Urban CHWs in the development and implementation of their action plans.
Conducts monthly supervisory meetings with Urban CHWs
Ensures regular replacement of supplies in the Urban CHW kits.
Conducts regular visits to the Urban CHWs in their communities to assess and assist their work.
Encourages team work among Urban CHWs, especially when they are working in the same catchment population

Health management Information System:

Ensures the quality of the Tally sheets and community maps maintained by the Urban CHWs and assists the Urban CHWs where needed.
Completes the appropriate Monthly Activity Reports (MAR) and completes the facility Monthly Aggregated Activity Report (MAAR) in coordination with the HF in charge.
Uses the reporting system and information received from Urban CHWs to monitor health condition and submits findings to the in charge person of HF.

Ability to communicate effectively both verbally and in writing, with a range of individuals and groups

Skills and Experience:

Graduate of high school. A university degree relevant to health is preferred, male or female (a female is preferred).
At least 1-2 relevant experience in BPHS and Afghanistan health system 
Respected, self-motivated resident of the local community
Strong communication skills.
Experienced in community development, previous PHC and management experience and being a Nurse or Midwife will be an advantage.
Good reading and writing skills in Dari or Pashto.

Skills:

Excellent facilitation and community mobilization skills
Having a deep knowledge of Afghan traditions and cultural norms
Ability and willingness to work at the communities
Ability to conduct mobilization session at community level

Education and Working Experience:

Graduate of high school. A university degree relevant to health is preferred, male or female (a female is preferred).

Languages Skills:

Fluency in spoken and written Dari and Pashtu is a requirement. English Language skills is a plus point.
