BACKGROUND:
The Ministry of Public Health is committed to providing high quality healthcare services to Afghans. Considering the increasing incidence of various types of cancers among Afghans and non-existence of a comprehensive cancer centre, MoPH decided to deliver cancer healthcare services to cancer patients through establishing a cancer department. Based on WHO estimates, nearly 20,000 Afghans suffer from various types of cancers each year while more than 15,000 die every year. The most common types of cancers are that of breast, oesophagus, stomach, lip/oral, cervix, and lung.
To address the increasing cancer incidence and deaths, the Ministry of Public Health established the Cancer Diagnosis and Treatment project in January 2015 which was promoted to National Cancer Control Program (NCCP) in January 2017. NCCP is addressing the problem through cancer primary prevention, early detection, diagnosis and treatment, palliative care, surveillance, and capacity building.
Since its inception, NCCP has conducted hundreds of awareness programs for health care providers as well as to lay people to increase the rate of early detection of various cancers. Moreover, NCCP created the first hospital based cancer registry in Afghanistan and working on establishing the population based cancer registry in Kabul. NCCP also provided training to the newly hired staff in and outside the country in various cancer care fields.
One of the main achievements of NCCP is the establishment of Cancer Center currently based in Jumhuriat Hospital. The 30-bed medical oncology unit is located on 9th floor while the 16-bed surgical oncology unit is located on 10th floor. In addition, the OPD, Walk-in-clinic and the 20-bed Day-Care are situated on second floor.
Between 70 and 100 patients visit the outpatient department each day. The patients include those who are already diagnosed with different types of cancer in other hospitals mainly undergoing chemotherapeutic and surgical treatments in the Cancer Center. Moreover, some patients visit the Cancer Center with advanced stage of their disease which require palliative care.
PURPOSE OF ASSIGNMENT
The post holder will provide specialist knowledge; undertake investigation and treatment in the diagnosis, management and clinical care of patients with Upper and Lower Gastrointestinal disorders.
JOB SUMMARY
To provide excellent medical care, and to actively participate in and contribute to the management of the Endoscopy Unit. Participate in the maintenance of endoscopes and other specialized equipment used in endoscopic and non-endoscopic procedures and provide technical assistance during such procedures.
RESPONSIBILITIES: Principal Duties and Tasks

Working within Hospital and Departmental guidance, policy and protocols undertake endoscopy procedures and be aware of the associated responsibilities and accountabilities. Clinical decision making skills and legal accountability must be understood together with the principles of precarious liability. -
Develop and maintain work plan of the endoscopy unit. -
Perform advanced and specialised clinical skills in assessment, diagnosis and treatment of endoscopy patients to enhance the patient/carer experience. To demonstrate critical thinking and reasoning skills in diagnostic decision-making -
Develop common standards and practices related to data/information collection. -
Administer conscious sedation/local anaesthesia within agreed protocols to patients requiring Endoscopy in line with the Nurse Prescribers course. Identify abnormal GI pathology, perform biopsies, photographs and complete pathology requests as required -
Perform advanced and specialised clinical skills in pre-assessment of colonoscopy patients -
Understand and implement the principles of informed consent and perform relevant consent procedures for approved endoscopy interventions. -
To provide appropriate guidance regarding wellness, lifestyle, disease risks and potential changes in health status, including health awareness, screening and prevention.
Qualifications & Requirements:

Bachelor’s in Medicine -
At Least Three Year Work Experience In Internal Medicine or Surgery -
At Least one Year Work Experience in Gastro Entrolgy or Certificate Of Endoscopy -
At least one year work experience in endoscopic procedures
SKILLS and KNOWLEDGE

Expert knowledge in anatomy and physiology of the GI tract -
Leadership of clinical governance and patient safety -
Able to perform diagnostic endoscopy -
Achievement of high quality patient care through evidenced based practice -
Communicates articulately and sensitively with good written English and able to demonstrate the ability to deal with distressing or emotional circumstances.
