Objective of the TA:

As a standardised package the TPHS specifies what services different types of tertiary level hospitals should offer.  It also lists the staff, equipment, diagnostic facilities, medications etc. required to provide these services. Thus, a key instrument has been made available which ensures that the most relevant and effective health interventions are offered and that good quality of services is uniformly provided in the central, speciality and regional hospitals of Afghanistan. As a consequence, referral, treatment and care of patients with complicated conditions will improve. This will not only have a beneficial impact on the health of the population, but may also imply some economic benefits, as those, who have so far been looking for treatment in foreign countries can now receive high quality treatment in Afghanistan. As regards the MoPH, the TPHS will also help with the budgeting of tertiary hospitals, improve monitoring and evaluation and facilitate the acquisition of donor support.

 

Main Tasks and Responsibilities:

Review/update the zero draft of the TPHS sub-packages developed in the TCPH-1.
In cooperation with the TPHS technical working group: Organize 2 consensus-building workshops on the TPHS document 0-draft.
Facilitate the consensus-building workshops and document the results.
Incorporate the recommendations from the consensus-building workshops in the TPHS document, fine-tune and edit the document
Support the organisation of a 2-days conference for presentation of the TPHS-document to international development partners and other relevant stakeholders;
Assist the stakeholder conference as a presenter/facilitator and document the conference results.
Considering the conference results: finalize a sound draft of the TPHS to be submitted to the MoPH for formal approval.
Upon request of DGCM: assist in the approval process of the TPHS document
Review the dari/pashto translation of the TPHS document
Conduct an orientation workshop on the TPHS for directors/managers of eligible hospitals
Throughout the process ensure good relationship and close cooperation with partners esp. in MoPH; coordinate with IPEHS-process and relevant development partners.

 

 

 

 

Deliverables:

2 consensus building workshops at central level (max. 20 participants each because of social distancing during Covid-19 pandemic) – 2 workshop reports.
 stakeholder conference (length and type – web-conference or physical presence – yet to be decided depending on COVID-19 restrictions) – conference report.
A final agreed version of the TPHS document – ready for official MoPH approval.
Orientation workshop on TPHS  for directors/managers of eligible hospitals (max. 20 participants – central level) – workshop report.

 

Reporting and Working Conditions

On a day-to-day basis, the NKE will report to the General Director, Curative Medicine, MoPH.

The NKE will report, contractually, to the Programme Team Leader.

The NKE will work with management and staff within the Ministry of Public Health, Kabul, Afghanistan.

The expert will provide TA to the MoPH staff by physical presence in the respective department, he will not work from home or telework.

Program Background:

The Technical Cooperation Programme to the Ministry of Public Health (TCPH) is funded by the European Union and implemented by a consortium of GIZ and m4health. The overall objective is to support the MoPH in the strengthening of its technical and managerial capacities to steer and manage the provision of health care services and to develop the Afghan public health system. In its 2nd phase (July 2020 – August 2022), the programme provides TA in the following major components: (1) Strengthening of Health System Governance and MoPH institutional development. This includes the review and updating of relevant policy and strategy documents, reform of the health care financing system, strengthening of aid coordination and support to SEHATMANDI. (2) Development of health services and human resources, covering areas such as clinical specialization, medical equipment and consumables management, hospital accreditation and the TPHS. (3) strengthening of public health programmes (NCD, cancer, mental health and disability) (4) monitoring and evaluation of health care services and systems performance with a special focus on data analysis and reporting as well as electronic medical record system.

This NKE assignment refers to TCPH Component 2: Development of Health Services and Human Resources, Result 2.2: Tertiary Package of Health Services, Activity: 2.2.1: Complement / finetune the sub-packages developed in the TCPH-1 project (source: 1st Annual Work Plan)

 

Rationale for TA

More than ten years ago, clearly defined standardised service packages had been introduced for the primary (BPHS) and secondary (EPHS) levels of the health care system. With the support of TCPH, a similar standardized service package is newly being established for the tertiary level. During TCPH I six different NKE assignments were already implemented within the framework of TPHS development. Based on an assessment of the scope and volumes of tertiary care level services currently provided at the Kabul central and specialty hospitals, and reference made to international good practice examples, key elements of a TPHS were defined and a plan elaborated in a consultative process. A  TPHS „ strategy and policy paper“ was developed, which includes recommendations for the general organization of tertiary level hospital care in Afghanistan as well as three standard sub-packages in the clinical specialties of: (a) Neonatology and  Referral Pediatric Care (b) Cardiovascular Diseases (c) Oncology. In a further step more specific information was added to the sub-packages and the document was restructured and harmonized with the BPHS and EPHS. By now – as a result of the above mentioned efforts – a comprehensive “TPHS Zero-Draft” is available. This is yet to be fine-tuned and presented to experts and the Key Stakeholders in a consensus-building process, –  with the objective to  finalize the TPHS document for formal MoPH approval. As a support to these remaining tasks further TA has been requested by the MoPH (referring to these TOR). . Finalization and approval of the TPHS document, however, is not an end in itself, but the necessary prerequisite for TPHS implementation, which is the main focus of TCPH activities during the current second phase of the project.   Additional TA will be required in order to support the rather complex tasks of  TPHS piloting and  implementation.

Qualifications, Skills and Experience

For this assignment a senior national NKE is required. He/she should have a medical background and have a good knowledge of hospital care at secondary and tertiary levels.  The candidate should also have excellent report-writing and facilitation skills and experience as a moderator/trainer. Excellent communication and networking skills are required as well as management competence.

Selection Criteria

Clinical background (MD)
Advanced academic degree, (public health or clinical specialization)
A minimum of 10 years professional experience in the health sector
At least 7 years working experience in the Afghanistan public health system
Proven experience as a facilitator/trainer (min. 1 year)
Dari/Pashto native speaker with very good command of written and spoken English (including English report writing skills);
Sound skills in MS Word, MS Excel and MS PowerPoint

 

Desirable:

Postgraduate degree, MPH or similar
Working experience in the MoPH
Working experience in a tertiary level hospital
The expert should have international experience in the development and implementation of TPHS (or similar)
